If the answer is 'Yes' please answer the following questions:
- Do you use an assistive technology such as a screen reader or voice recognition software?
- Do you avoid using the mouse?
- Do you find it very difficult to use a mouse?
- Do you find it hard to see the screen?
- Do you experience pain or fatigue when using the computer?
- Do you have difficulties reading?
- Do you have a learning disability or cognitive impairment?
